How to Obtain Medical Records

You can request a copy of medical records for

  • yourself
  • your child's (under the age of 18)
  • an individual whom you are their legal guardian

If you are requesting a copy of your own medical record for yourself or to be sent to someone other than yourself, please complete the Authorization for Use or Disclosure of Health Information.

If you are requesting medical records for an individual of whom you are their legal guardian, please provide copies of guardianship papers along with the Authorization for Disclosure of Patient Health Information and mail to:

Birth Certificates and Death Certificates
Birth and Death certificates can be obtained from the Washtenaw County Clerk's Office.
